Silvi Kirilov was born on December 17, 1950, in the village of Slavotin, according to the National Assembly's website.

He graduated from the Higher Medical Institute specializing in urology and further specialized in Moscow and Göttingen. Silvi Kirilov holds a Doctor of Medical Sciences degree and a Master's degree in healthcare economics from the University of National and World Economy, with a focus on health management, according to his party’s There Is Such a People website.

Since 2002, Kirilov has held various leadership positions at the Sofia-based Alexandrovska University Hospital. In 2007 he was appointed Executive Director of the Doverie Hospital in Sofia. In 2011, he headed the Executive Agency for Transplantation, followed by roles as Executive Director of Health Insurance Fund Europe AD and Uni Hospital in Panagyurishte.

He has served as an MP in the 45th, 46th, 47th, 50th, and 51st National Assemblies. As the oldest MP, he opened the inaugural sessions of the 47th, 50th, and 51st National Assemblies.
